About Us
EverWell Collaborative Solutions, PLLC is a team of Psychiatric Mental Health Nurse Practitioners dedicated to improving access to high-quality behavioral health care for individuals living in nursing homes, assisted living facilities, and long-term care settings.
We understand that mental health needs in these environments are often complex and require a thoughtful, collaborative approach. Our providers work closely with facility staff, primary care teams, families, and interdisciplinary care teams to ensure residents receive compassionate, individualized psychiatric care.
Our services include psychiatric evaluations, medication management, behavioral health support, and ongoing follow-up visits designed to improve quality of life, emotional well-being, and overall functioning.

Psychiatric Evaluation and Medication Management
Comprehensive psychiatric assessments and ongoing medication management tailored to the unique needs of geriatric and medically complex residents.
Psychotherapy and Behavioral Support
Supportive psychotherapy and behavioral interventions to help residents manage depression, anxiety, trauma, cognitive decline, and other mental health conditions common in long-term care.
On-Call Support for Acute Behavioral Concerns
Our providers are available for consultation regarding acute behavioral changes, agitation, anxiety, or psychiatric decompensation, helping facilities manage situations quickly and safely while reducing unnecessary hospital transfers.
Psychotropic Medication Monitoring and GDR Compliance
We partner closely with facilities to ensure compliance with CMS and regulatory guidelines regarding psychotropic medications. Our team conducts monthly psychopharmacology reviews and Gradual Dose Reduction (GDR) monitoring, helping facilities appropriately evaluate and adjust medications while maintaining resident stability.
Interdisciplinary Collaboration
We work closely with nursing staff, medical providers, social services, and administration to create collaborative treatment plans that support both resident wellbeing and facility goals.
Staff Education and Behavioral Guidance
Our team provides guidance and education to staff on behavioral management strategies, psychiatric symptoms, and medication considerations, empowering caregivers with tools to support residents more effectively.
